Family Connections has four programs to serve pregnant women, families, and their babies.

Purpose
Baby with Block
  • To enhance parenting and life skills 
  • To provide families with other community resources 
  • To identify and refer any potential physical and developmental concerns
Becoming a Mom
  • Services Provided:
    • Six prenatal education classes

Maternal and Infant Health Program  
  • Services Provided: 
    • For pregnant women and infants 0-12 months 
    • Social worker to offer community resources and referral services 
    • Assistance with obtaining health insurance and a medical home 
    • Monthly visits with a registered nurse 
      • Physical assessments
      • Breastfeeding guidance and resources – New!  Flint Hills Breastfeeding Resource Guide
      • Education about healthy pregnancy and infant development 
      • Referrals to additional community resources
Newborn Home Visit 
  • Services Provided: 
      Baby in Car Seat
    • For infants 0-2 months 
    • One or two home visits with a registered nurse
    • Infant will receive a nursing assessment including weight check
    • Information about infant care and safety 
    • Referrals to additional community resources
Home Visiting Program 
  • Services Provided:
    • For families interested in a higher level of support 
    • Pregnant women and families with children 0-3 years 
    • Weekly home visits with a trained registered nurse or licensed social worker 
    • Emphasis on enhanced parenting skills 
    • Education about pregnancy and early childhood development 
    • Referrals to additional community resources
Payment
  • Services are provided at no cost to families
Most services by appointment, but can accommodate walk-in clients at times
